1Some services require prior authorization (PA). Our network providers know what we cover under your benefit plan. They also know what requires prior authorization and will request approval from Health New England on your behalf. 2Health New England additional benefits include allowances and reimbursements that must be used within the one or two calendar year period, as well as other additional benefits. Refer to the Summary of Benefits or call Member Services if you have questions about what items and services are covered.
3You must see a TruHearing® provider to use this benefit. Please note, hearing aids purchased through other providers are not covered. Other providers are available in our network.
4Mail Order: During the coverage gap stage, for the Health New England Premium (HMO) plan, preferred generics are covered at $8 for a three-month supply. Generics are covered at 25%, and Preferred Brand and Non-Preferred Drugs are covered at 25% of the price or the Health New England negotiated price, whichever is lower. For the Plus (HMO), Choice (HMO), Value (HMO), Compass (PPO) and Compass Premier (PPO) plans, the standard coverage gap cost sharing applies. During the catastrophic coverage stage, standard catastrophic coverage applies for all plans.

6Out-of-network/non-contracted providers are under no obligation to treat Health New England Medicare Advantage members, except in emergency situations. Please call our Member Services number or see your Evidence of Coverage for more information, including the cost sharing that applies to out-of-network services.
7The OTC benefit can only be redeemed through FirstLine Benefits. Quarterly credits expire every three months and do not accumulate.
